SURFBOARD & WETSUIT RENTALS AT CARCAVELOS BEACH, LISBON

I, the undersigned, acknowledge that I wish to practice Surfing or BodyBoarding at Carcavelos Surf School (CSS), located at Praia de Carcavelos, Av. Marginal, 2775-604 Carcavelos, Cascais, Portugal. Carcavelos Surf School is operated by the company Sunset Chapter, Lda registered and located at Rua 5 de Outubro N.º16, 2775-562 Carcavelos, Cascais, Portugal.

By signing this term, I declare that I understand all the risks inherent in the practice of Surfing or BodyBoarding, and these risks include, but are not limited to, injuries and physical damage to myself or third parties. I further acknowledge that Sunset Chapter, Lda and Carcavelos Surf School are not responsible for any damages that may occur, releasing and fully dispensing with Sunset Chapter, Lda and Carcavelos Surf School, from all liability related to my practice and, during the period in which I am practicing the modality of Surf or Bodyboard with the technical equipment rented by me.

I further declare that I will take care of the material delivered to me by Carcavelos Surf School, which must be returned in the same conditions in which it was delivered to me at the time of rental, otherwise I will have to bear the costs inherent to its repair/replacement if it is damaged.

I also declare here that: - Under the terms and for the purposes of the provisions of Law No. 5/2007 of 16 January, on the basis of physical activity and sport, I declare that I have no contraindications for the practice of sporting activity, ensuring that I have the necessary physical fitness and that I am in good physical condition to perform water and impact sports, namely Surfing or Bodyboarding;
- I don't have any kind of heart problems;
- I have not consumed, nor am I under the influence of, any substances such as alcohol, drugs or medication;
- I am not in a physical (i.e. pregnant) or psychological situation that in any way prevents me, harms me or for which the practice of Surfing/Bodyboarding is not recommended.
